Middletown Wellness Wheels
Middletown Wellness Wheels is a local transportation program managed by the Middletown Township Health Department that offers free rides to local health- and wellness-related destinations for Middletown residents with substance use disorders, those who are at risk of substance use disorders, or those who need access to mental health services. In partnership with Uber Health, Middletown aims to encourage and support recovery efforts where personal transportation may be a barrier to accessing care and services.

Ride requests can be made by Middletown residents with pick-up and drop-off locations within a 15-mile driving radius of Middletown Town Hall (1 Kings Highway). Distances are confirmed at the time of the appointment. Rides beyond the 15-mile driving distance will be declined.

- Urgent Care facilities
- Primary and specialist physician offices
- Recovery facilities
- Wellness centers
- Locations offering social support services
- Mental health services
- Dentists
- Pharmacies
*Destination points of private residences are not permitted.
This program is meant to assist Middletown residents in substance use recovery efforts and is therefore not open to the general public for all rides similar to the use of the generic Uber app.
Ride Guidelines
- Both one-way and return trips are scheduled simultaneously via the Ride Line.
- No ride modifications can be made once the rider has “accepted” the ride.
- Stops made in between the pre-planned drop-off and pick-up locations are not permitted.
- The sharing of rides with other individuals (aside from the rider’s children) is not permitted – all riders must make appointments separately with the Ride Line.
- Tipping of drivers is not permitted.
- Special accommodations, such as wheelchair accessibility, must be explicitly requested at the time the appointment is made with the Ride Line due to limited capacity for our geographic region. The Ride Line will do its best to accommodate all special requests that are fully expressed at the time of the appointment.
Car/booster seats are not provided. Requirements are as follows:
NJ Car Seat Guidelines
- New Jersey law requires children to use car seats in all passenger vehicles with seat belts, including ride-sharing vehicles like Ubers.
- Children under 8 years old or 57 inches tall must be secured in the back seat of a motor vehicle.
- Children under 2 years old and 30 pounds must be secured in a rear-facing seat with a 5-point harness.
- Children 12 years old and under should ride in the back seat.
- Drivers can cancel a ride if a child doesn't meet the height and weight requirements for a car seat, or if the driver thinks the child can't be safely transported in the seat.
The Middletown Township Health Department reserves the right to deny ride requests that do not meet the parameters of this program or for services that are not available in our town for the time requested, i.e. wheelchair accessibility.

Middletown residents can request a ride via the Ride Line during business hours of Monday through Friday, 9:00 AM to 3:00 PM. All rides are pre-planned through personnel at the Middletown Township Health Department and require a request be made via telephone with the Wellness Wheels Ride Line at a minimum of 5 business days in advance.
The Ride Line is not answered after hours, on weekends, or on holidays. There is no ability to leave a voicemail message on this line because all ride requests require live, verbal confirmation from Health Department staff.
If a cell phone number with texting capabilities is given with your appointment, you will receive a text confirmation.
If a landline phone number is given, you will receive an automated call confirmation.
Please note that there will be personal information, such as full name, address, and phone number, required to be given at the time of the ride request and will be stored in Uber Health's HIPAA-enabled web-based dashboard. For more information, read Uber’s Privacy Notice.
