Clinic Details
- The clinic is only for Middletown residents ages 6 months and older. It will take place in the Training Room on the first floor of Town Hall (1 Kings Highway).
- A completed consent form is required. You can either complete this fillable PDF or fill it out in person at the clinic.
- The main entrance to Town Hall is facing Kings Highway. Parking will be available at Town Hall's parking lot, located off Route 35 behind the building. There will be signs for the clinic at Town Hall.

Pre-Registration Required
The last day to pre-register is Tuesday, October 15th.
