All dogs must be on a leash under the control of an adult. If you would like to apply for your dog’s 2018 license, please bring your renewal paperwork along with cash or a check for the appropriate amount. Credit cards cannot be accepted at the clinic site, to use a credit/debit card, you must pay online or in our office. We will now be offering a Pet Microchipping program at our rabies clinic. This will only be offered to Middletown residents with a current pet license. The Microchips will cost $10.00 per pet. This fee includes the chipping of your dog as well as lifetime registration in a microchip database. If you are interested in obtaining a Microchip for your pet please bring your licensing paperwork, along with a check or cash to pay for your license and microchip, to the rabies clinic.
