Vital Statistics is the collection of population information relating to birth records, death records, & marriage records.
As of February 2007, Middletown Township will be allowing applications for civil union licenses. Domestic partnership license applications are currently available.

According to NJ state law, no requests for vital statistic information may be honored over the phone. All requests must be made via the downloadable Vital Statistics Request form.

Births Occurring over 80 years ago, marriages occurring over 50 years ago, and deaths occurring over 40 years ago are considered Genealogical Records. To request Genealogical Records, you must contact the State of New Jersey directly.
