The Registrar's Office hours for applying for marriage, civil union, domestic partnership/ remarriage or reaffirmation of a civil union and all certified copies of vital records requests are performed BY APPOINTMENT ONLY.  Please allow at least a minimum of one month with a maximum of 3 to 6 months before the scheduled event for a marriage/civil union/remarriage or reaffirmation of a civil union or domestic partnership application in order to obtain an appointment to ensure there are no conflicts with your intended ceremony day.

Vital Statistics is the collection of population information relating to birth records, death records, marriage records, civil unions and domestic partnerships.

Requests for Vital Statistic information will not be honored over the phone. All requests must be made via the downloadable Vital Statistics Request form. Proof of valid identification and address must be submitted with the completed application. Certified copies of marriage, domestic partnership, birth, & civil union records are $20.00 each payable by money order or certified bank check to the: Township of Middletown. Certified copies of death records are $25.00 for the first copy and $2.00 per each additional document.  Note: There is a $25.00 fee for each correction to a vital record.

A completed application, including proof of valid identification (unexpired photo current driver's license along with proof of address) and a money order or certified bank check for all fees that apply may be mailed or physically submitted to the Health Department Office (BY APPOINTMENT ONLY AT THE OFFICE). If your identification has a previous marriage name please call the office for further instructions.
