The Registrar's Office hours for applying for marriage, civil union, domestic partnership and all certified copies of vital records to be performed by appointment only.  Please allow at least a minimum of one month with a maximum of 3 to 6 months before the scheduled event for a marriage/civil union or domestic partnership application in order to obtain an appointment to ensure there are no conflicts with your intended ceremony day.

Vital Statistics is the collection of population information relating to birth records, death records, & marriage records.

Requests for Vital Statistic information will not be honored over the phone. All requests must be made via the downloadable Vital Statistics Request form. Proof of valid identification must be submitted with the completed application. Certified copies of marriage, domestic partnership, birth, & death records are $15.00 each. Note: There is a $25.00 fee for each correction to a vital record.

A completed application, including proof of valid identification and a check for all fees that apply may be mailed or physically submitted to the Health Department Office.
