The Registrar's Office hours for applying for marriage, civil union, domestic partnership and all certified copies of vital records  to be performed by appointment only.  Please allow at least a minimum of one month with a maximum of 3 to 6 months before the scheduled event for a marriage/civil union or domestic partnership application in order to obtain an appointment to ensure there are no conflicts with your intended ceremony day.

Marriage, Birth or Death records may be obtained from the Middletown Township Health Department, only if the event occurred within the Township.

You may apply for marriage, birth or death records by downloading and completing the Vital Statistics Request Form. Vital Statistics and Records information requests will not be honored via telephone.
