Birth/Death Records

Note: The Registrar's Office hours for applying for marriage, civil union, domestic partnership and all certified copies of vital records or inquiries to be performed during the hours of 9:00 AM - 12:00 PM and 2:00 PM - 4:00 PM.

You must call and make an appointment for all Registrar of Vital Statistics documents until further notice. Walk-ins are no longer accepted at this time.

Birth and/or Death records may be obtained from the Middletown Township Health Department, only if the event occurred within the Township.

You may apply for birth and/or death records by downloading and completing the Vital Statistics Request Form. Vital Statistics and Records information requests will not be honored via telephone.
